Late Registration Birth Certificate for Japan Visa

Hi. I have late registration on my birth certificate. The agency asked me to submit my baptisimal certificate and Form 137.

Question are:

  • Pwede kaya TOR instead of Form 137?
  • I already have a baptisimal certificate, but this was issued a long time ago. Should I get another copy?
