Anyone else feel like the NYU required health history form is a little too invasive?

This is much more invasive than the one I had to fill out for undergrad. I am a CSA survivor and quite frankly I'm not comfortable disclosing my sexual assault history on a law school form. I'm not sure why they need this information. On the other hand, lying/omitting on the form also makes me uncomfortable. I'm not sure how realistic either of these fears is, but I worry if I disclose this could somehow get me labeled as a risk of some sort, but if I don't disclose now but then later do for some reason, that could be seen as an inconsistency.

Regardless, it seems really weird of them to ask. Is anyone else struggling with filling out these forms?
