No pain no gain skill on DB

Does anyone know if "No pain no gain" skill works when in wraith mode. I see that I take the extra damage, but not sure about if I'm dealing the extra damage.

Anyways for farming rams I figured out that not having the skill is better for now since I don't need the damage and taking the extra damage can kill me at times.
