How does something like that happen?

I saw this post going viral locally, and I am trying to think how something like this can happen? Just genuinely asking. It was a blended drink. I’m trying to think at what period during the process of making this drink would this have been missed or could’ve happened? There was also a screen shot of her email and date to the company….Thoughts?
