should it be fully clear by now?

I’ve been on spiro for around 8-9 months, first photo is a year ago before spiro and second is today. i still get a lot of mild breakouts. my derm is now pushing accutane again, and i have a month to decide. should i keep going with spiro or bite the bullet with accutane?
