Understanding Settings, wtf am I doing?
Hi. I'm just looking for some guidance.
Background:
I have "mild" sleep apnea with a 24 RDI, so lots of RERAs. I've developed asymmetric septal hypertrophy and left ventricular relaxation syndrome at the age of 35 directly from untreated apnea so nothing about this is mild at all.
Things I've tried:
- CPAP/APAP - did not treat me at all. Totally ineffective.
- Could not put pressures up high enough to make a difference without horrible aerophagia.
- Mandibular advancement device - I'm class 3 so this didn't really help.
- Full Face Mask - squished nose, cannot breathe.
- Combo FF and pillows mask - not as effective as nasal only mask.
Things I'm doing:
Cervical Collar
Nasal mask
Tongue Retainer
Sleeping on incline
Sleeping on side only
BIPAP - 20 min ramp.
Pressure Strategy:
I'm upping the ipap and epap by .2 every night to see if I can approach a therapeutic pressure for myself where I don't keep experiencing apneas and RERAs and all the other arousals. I find if I keep the PS at 1.2 so for example, Ipap is now set to 9 and epap is set to 7.8 - then I don't get aerophagia.
Question:
Is there some sort of catch with this strategy? Is it okay for IPAP and EPAP to be that close? I was thinking that it would help stent the airway open and since it's not a huge change in pressure it's not pushing past my weak lower esophageal sphincter so I'm not getting aerophagia but there's so much I don't understand about this treatment that I want to make sure this is a good road to continue down, just keep upping the pressure .2 every night and analyzing my data each morning to see if the flow rate has improved??
Anyone have any other suggestions?
