First therapy appointment

I have my first therapist appointment next week. I've tried to get therapy several times in the past & it always didn't really work out for various reasons. What should I plan to talk to about? How should I start? My issues are with some pretty serious depression that's gotten harder to cope with since getting sober, a difficult relationship with my mother based on me being really hung up on past hurts I can't seem to get over, that I feel weigh on me disproportionately to how bad they actually objectively were, & a past romantic relationship that I feel really weird about & hung up on, & that I have really profound fear of driving over bridges & every time I'm in a car on a bridge I'm "sure" I'm going to die. Any advice would be appreciated. Thank you.
